Oral Assessment and Treatment Consent Form

We have scheduled your pet for an Oral Assessment and Treatment and/or oral surgery. This procedure involves anesthesia, digital dental radiographs, a comprehensive oral exam, scaling/polishing of healthy teeth, and possible oral surgery to remove diseased teeth.
  • Your pet will be undergoing anesthesia. To improve safety and prevent vomiting you will need to fast your pet. Please pick up any food and fast your pet after 8pm the day before surgery. You may leave water available to your pet. If you pet is on any medications, please ask us for instructions on dosing the day of surgery. Please enter any medical conditions your pet has or known allergies below.
  • Pre-Operative Bloodwork

    Your pet is undergoing general anesthesia today. An exam will be performed prior to anesthesia and monitoring will be done by a technician during anesthesia. We perform a mini blood profile prior to anesthesia to help screen for any pre-existing conditions that my not be evident on physical exam. This profile includes a CBC, liver and kidney values, and blood glucose. If your pet has had appropriate bloodwork done within the last 30 days, this profile may be waived at the doctor's discretion.
  • For blood pressure support and better care while under anesthesia, an intravenous catheter will be placed. This allows delivery of fluids and medications during the procedure and helps reduce anesthetic risks. A small patch of hair will be shaved to permit disinfection of the catheter site. A wrap may be placed on your pet's leg after catheter removal. If this wrap is in place at discharge, please removed the wrap once your pet is home.
  • If fleas are found on my pet while in the hospital, they will be treated at my expense.
  • It is very difficult to know prior to a full examination and radiographs under anesthesia the true extent of oral disease. Often more treatment is needed than is found during the original exam when the treatment plan is created. Therefore we offer three options for you to choose from below. Please review each option carefully and select the one that best meets your needs. Make only one selection.
  • Select one of the Following Three Options:

  • I understand that during the oral exam it may be found that teeth are diseased to the extent that they cannot be saved. In this case, the doctor may elect to extract the tooth/teeth with oral surgery. If a mouth is badly diseased, many teeth may be extracted. There are additional charges for oral surgery.
    Antibiotics and pain medications may also be prescribed for an additional charge.
    I also authorize the use of appropriate anesthetics and other medications, and I understand that the hospital support personnel will be employed as deemed necessary by the veterinarian. I have been advised as to the nature of the procedure(s) or operation(s) and the risks involved. I realize that the results cannot be guaranteed.
