I understand that during the oral exam it may be found that teeth are diseased to the extent that they cannot be saved. In this case, the doctor may elect to extract the tooth/teeth with oral surgery. If a mouth is badly diseased, many teeth may be extracted. There are additional charges for oral surgery.
Antibiotics and pain medications may also be prescribed for an additional charge.
I also authorize the use of appropriate anesthetics and other medications, and I understand that the hospital support personnel will be employed as deemed necessary by the veterinarian. I have been advised as to the nature of the procedure(s) or operation(s) and the risks involved. I realize that the results cannot be guaranteed.
