Surgery/Anesthesia Consent Form

  • Please list the procedure your pet is scheduled for. If needed we can assist in filling out this information.
  • Your pet will be undergoing anesthesia. To improve safety and prevent vomiting you will need to fast your pet. Please pick up any food and fast your pet after 8pm the day before surgery. You may leave water available to your pet. If you pet is on any medications, please ask us for instructions on dosing the day of surgery. Please enter any medical conditions your pet has or known allergies below.
  • We require an IV catheter be placed for all pets undergoing general anesthesia. This allows delivery of fluids and medication during the procedure, which helps to reduce anesthetic risks. A small patch of hair will be shaved to permit disinfection of the catheter site. Your pet may have a small bandage on their leg when going home. Make sure to remove the bandage once they are home. An exam will be performed on your pet prior to anesthesia, and monitoring will be done by a technician during anesthesia. To screen patients for any pre-existing disease that may not be evident on physical exam, we recommend pre-anesthetic bloodwork that includes a mini Chemistry profile and a CBC. This bloodwork is required for all pets over 7 years of age, but is recommended for all pets.

  • Pets arriving with fleas present a hazard to all other patients in the hospital. Please be advised that if fleas are found on your pet while in the hospital, they will be treated at your expense.
  • Pain Relief

    Recognizing and controlling pain can help reduce stress and improve recovery times. In addition to the normal pain control provided by anesthesia, we may perform post-operative laser therapy and may give your pet post-operative injections to provide comfort during recovery. We may also send home medications to provide ongoing pain control. Your veterinarian will choose appropriate pain relief medication as they deem necessary. There may be an additional fee for this medication if not already included in the treatment plan.
  • I understand that during the performance of the procedure(s) described above, unforeseen conditions may arise. Therefore, I hereby consent to and authorize the performance of such procedures as are necessary and desirable in the exercise of the veterinarian's professional judgement. I also do hereby acknowledge that I have been advised of the nature of the risks involved and understand that there are no guarantees either expressed or implied that the procedures authorized will be without complications from unexpected events beyond the veterinarian's and hospital's control.
  • I am the owner or agent of the above described animal and have the authority to execute this consent. I hereby
    consent and authorize the performance of the afore mentioned procedure(s) or operations.
